Frequently Asked Questions

  • What are the changes happening?
    Starting April 5, 2024 Tim Sayles, PA-C will be transitioning, exclusively to a personalized-style medical practice. He will be join Drs. Delores and Mark Musco at FOCUSEDMD™.
  • Is the entire practice moving to a subscription model?
    No. The remainder of the providers at Diablo Family Physicians will be practicing in a traditional insurance based model of care. There is no need to leave the practice as the team of medical providers will have the capacity to continue caring for you at our current location.
  • Is Tim Sayles, PA-C leaving Diablo Family Physicians?
    Tim Sayles, PA-C will continue to practice in the same location as Diablo Family Physicians. He will transition to this new model of care April 5th, 2024 and will be seeing patients under that higher connected care model at that time.
  • Will I be able to see other FOCUSEDMD™ providers or Drs. Musco.
    The FocusedMD program is a panel based subscription model. If you sign up with Tim Sayles, PA-C he will be your primary care provider in the practice who will see you and coordinate your care as needed. Generally speaking, your clinical relationship and care will be provided solely by Tim Sayles. There may be som exceptions such as coverage during vacations or when Physicians are needed from time to time for supervisory or consultative reasons. Additionally, the patients who are enrolled up with Drs. Muscos will continue to be seen by their assigned provider.
  • Is the practice closing or moving?
    No. The practice will continue at its current location with the current group of providers that you know and appreciate and will continue to be under the management of Bass Medical Group.
  • How does the subscription program work?
    Tim Sayles, PA-C decided to transition the practice into a subscription-based model. This model allows Tim Sayles, PA-C to focus on the needs of their patients and become their partner for all of their health and wellness needs. Patients that wish to remain under their care will subscribe to the plan for a monthly or annual fee.
  • How do I benefit from joining this plan?
    In return for your enrollment to FOCUSEDMD™ you will be receiving many benefits, including: Convenient Appointment Times. Due to our lower patient panel size and detachment from healthcare insurance plan restrictions, you will experience rapid scheduling and office visits unhurried by plan requirements. Complete health perspective. Your Provider now has the ability to work closely with you throughout the entire course of any health issues, including: follow up with your specialists, discussing results from tests or diagnostic procedures, reviewing additional treatment options, admissions to the hospital for a surgical procedure, and more. More convenient direct contact: Our ongoing routine exams support communications will provide you 24/7 immediate care communication connection all year. You can contact your provider directly via phone, or secure text, and your provider will respond promptly.
  • How did you come up with the pricing?
    Our goal is to make the program fee affordable for patients and their families, while providing proactive routine wellness exams supported by modern 24/7 communication. Due to the reduction in our patient panel size our patients experience longer appointments, increased access and more comprehensive care.   • I can’t afford to pay the fee, what should I do?
    We offer several different payment options; our Team can speak with you personally to find a solution that fits your needs. For those patients who are not interested in investing in this higher-focused care model, we understand and thank you for trusting us with your care all these years. We will attempt to transition your care from Tim Sayles, PA-C, to other available medical providers at Diablo Family Physicians. Our team will be expanding to support the patients who decide to stay after Tim's departure. These outstanding medical providers will continue to be dedicated to delivering the highest quality; traditional insurance-funded care possible.   • What if I decide, in several months, that I don’t want to stick with the program? What happens next?
    Given the demand, it’ s smart to sign up now. If, after a period of time, you decide the program isn’t right for you, you can terminate with a 30 day written notice anytime after the first 90 days of membership. However, we are asking our patients to commit to a minimum of 12 months.
  • I don’t want to join this program. I need to find another Provider. Can you help me?
    Yes, we have recently hired a new physician for our practice, Dr. Gurpreet Boparai, and she is currently accepting new patients. For those patients who are not interested in investing in this higher-focused care model, we understand and thank you for trusting us with your care all these years. We will attempt to transition your care from Tim Sayles, PA-c to other available medical providers at Diablo Family Physicians. These outstanding medical providers will continue to be dedicated to delivering the highest quality; traditional insurance-funded care possible.   • When do I pay the subscription fee?
    We offer two payment options: monthly or annually. You will not be charged until your program begins. Fees are charged on the 5th or 20th of each month should you elect to pay monthly. Our new subscription program will launch on April 5th, 2024.
  • How many patients is my Provider planning to take on with this high care model?
    Tim Sayles, PA-C is caring for over 3,000 patients currently. We anticipate each provider will have no more than a few hundred patients enrolled in each of their respective patient panels. Your provider is committed to limiting the total size of the practice, so that patient are able to enjoy the benefits of the program. It’ s another reason why you should sign up quickly, as the provider is accepting patients on a first-come/first serve basis.
  • Are there rules and restrictions about contacting the care team?
    One of the great benefits of this program is that it’ s actually much easier to connect with your provider- especially in the manner that you prefer – by text, video, in person, phone, or patient portal. Your provider will be available after hours or on weekends for urgent or emergency situations. All other inquiries would be responded to on the next business day. At this time we are not offering home visits to our patients. Additionally, our providers do not provide any services at the hospital.
  • If too many patients sign up for the membership, how will my provider care for everyone?
    Your provider is committed to limiting the total size of the practice, so that patients are able to enjoy the benefits of the program. It’ s another reason why you should sign up quickly, as the doctor is accepting program applications on a first-come/first serve basis. Patients who decided FOCUSEDMD is not for them can continue to see the team at Diablo Family Medicine.
  • What happens when my provider goes on vacation?
    Your provider will have on-call coverage with another Practitioner, so that if an urgent need arises, you’ll still get the care you need.
  • What happens when I need a specialist?
    First, you’ll see your primary care provider to discuss your health issue (same as your current situation). If needed, your provider will refer you to a specialist. What’ s different is that your provider is able to remain actively involved in your health issue. The provider can follow up with the specialist, is available to discuss your treatment options, and can work with you to schedule follow-up visits and/or coordinate discussions with the specialist. Having your primary care provider, a trained clinician who knows your entire health situation in detail, at your side as an active advocate can be enormously comforting and helpful to you, as you make important medical decisions.
  • How does the membership fee work with my HSA/FSA/HRA?
    FOCUSEDMD designed their private fees to be eligible medical expenses such that HSA/FSA/HRA funds are eligible to pay those fees. That said, check with your tax expert regarding your HSA fees, and check with your FSA/HRA plan administrator, to ensure eligibility: tax situations/plans may vary. Please note that each plan is individual designed by the administrator or employer and may not approve FOCUSEDMD subscription as an eligible expense. We can not guarantee nor are we responsible for approval. Please find the attached document for your convenience.
  • Is membership tax-deductible?
    While FocusedMD designed their private fees to be eligible medical expenses, the US tax code does not normally allow for medical expense deduction.
  • Can I submit my FOCUSEDMD™ fees to a healthcare insurance plan for reimbursement?
    No, FOCUSEDMD™ private fee services are designed to be for services entirely outside of Medicare or any private plan coverage. You must not submit any FOCUSEDMD™ fees to Medicare for reimbursement, and Medicare will not reimburse any portion of those fees. And, similarly, private plans will also decline to reimburse FOCUSEDMD™ fees because those fees are for services designed to be entirely outside of and in addition to the services plans cover.
  • If I am admitted to the hospital or need care at the home will the Providers do Hospital or home visits?
    No. The Providers are not privileged for in-hospital care currently. When you are hospitalized the team of providers that staff the hospital will be assigned to you by the hospital and they will directly manage your care during your stay. The providers will work closely with the doctors caring for you in the hospital to ensure good continuity of your care in and out of the hospital. The providers will not be offering home visits at this time. The Providers will be able to provide video based visits while you are at home to efficiently address your issues as they come up.
  • Is the practice moving?
    No, the practice is not moving locations nor are we closing. Diablo Family Physicians will still actively see current patients and new patients that choose not to invest in this personalized care model.
  • What will happen if I am not able to join FOCUSEDMD™?
    Not to worry. We have several skilled practitioners that can support you in your healthcare needs, in addition to welcoming our newest physician, Gurpreet Boparai, MD!
  • What if I have an HMO plan?
    Unfortunately, if your insurance is considered an HMO plan, you will not be eligible to join FOCUSEDMD. The providers at Diablo Family Physicians will be able to continue to care for you.   • What if I have a Medicare Advantage PPO plan?
    You have options! There are no plan restrictions, and you will be able to either join FOCUSEDMD™ or continue to have your care with the other providers at the practice.
